The undersigned spokesperson further attests that The Act meets the eligibility requirements included in the Official River City Ohio Blues Competition (hereafter referred to as the Competition) Rules.
It is agreed by the spokesperson that he/she and each member of The Act, do hereby exempt for all liability, The Blues, Jazz & Folk Music Society, Inc. (hereafter referred to as BJFM) and all person, organizations and companies that are directly or indirectly associated with the Competition.
It is further agreed by the spokesperson that he/she and each member of The Act do hereby accept full responsibility for all taxes and restrictions relating to contest prizes received by The Act; further, each member of The Act, listed on this form, herewithin permits BJFM the right to make use of his/her/their names, voices, pictures, visages and other likenesses of The Act for the purpose of advertising, publishing and promoting the Competition through media, including entertainment and music syndicated radio, film and/or television shows presented or produced by BJFM or its partners or assigns. In addition, the spokesperson and each member of The Act hereby agrees that BJFM will retain total ownership of the program(s), the right to edit the program(s), the right to broadcast the program(s), the right to copyright the program(s) and the right to license others to use these rights.

The following questions and material requirements are an integral part of this event and must be complied with. (READ CAREFULLY)

Stage/Backline equipment provided: (A.) Due to the growth and format change of this event , guitar players will now be required to use only the bass, and guitar amps provided ! But can still use their own instruments and effect pedals. (B.) Harmonica players: Will be allowed to use their own amps or the amps provided. (C.) Keyboard players: We do not provide keyboards or keyboard stands, so you may use your own. For those wanting to use Hammonds, it is your responsibility for the moving and placement to and from the stage area, both before your performance and after. (D.) Drummers: Must use the set provided! No changing of cymbals or snare drum. Will allow changing of bass drum pedal, only! You will be allowed to make adjustments for your playing style. ( height, cymbal angles, etc.) Due to the layout and size of the venue, these changes must be made to relieve the congestion and confusion around the stage area. To make sure the transition between acts goes as smooth as possible.

BJFM will provide:

1.) 5pc Black ‘Pearl’- Export Series single bass drum set w/heavy duty hardware, Zildjian & Sabian cymbals & all Evans heads. D. W. single bass drum pedal.
2.) ‘Hartke 3500 ’- bass amp w / compression & graphics. With 2 cabinets. Each w / 4-10’ speakers. ( 4 – aluminum & 4 – regular )
3.) Keyboard players: May run direct thru the P.A. system or will allow keyboard players to use their own keyboard amp. ( Guitar amps )
4.) 1 – 90 watt / DC 5 (dual caliber) Mesa Boogie combo single 12” plus 1-12”extension cabinet.
5.) 2 or more ‘Category 5’- all tube, handwired, point to point, Class A guitar amps. 1 – ‘Tsunami’/ “True” 18 watt / 1 –12”spkr. Plus 1 – ‘Ivan’ / “True” 20 watt / 1 – 12”spkr. ( dual channel amps) For info about ‘Category 5’ amps. ( see: www.category5amps.com )
6.) Stage lighting and PA system w/ in ‘your – face’ monitoring , direct boxes , microphones & stands will be provided..

Application must be received by Friday January 8, 2011. Bands will be notified of acceptance or rejection two weeks after that date after a review of the demo tapes by the Board of BJFM.
